What is REQUIRED to Enter:

1) PARENT OR ADULT: State Issued Photo ID

2) CHILD: For each child, a birth certificate or Medicaid Card

3) Proof of Address for 43206 or 43207 zip code. Examples are Driver’s License or CURRENT utility or pay stub.

 

How it Works:

1) Line up and enter through the south door/Community Kitchen Entrance.

2) You will register and be given a number. Please do not lose this number you will need this.

 

What You Need to Know:

1) Maximum of 4 children per family, Grades K – 12th only. This allows us to serve more families.

2) Absolutely no exiting and re-entering. Anyone who exits will have to go to the back of the line.

3) Children do not need to be present to receive school supplies.
